Child Assist Postal Registrations

If registering online isn’t for you, you can always fill out an application form by following these steps.

• 1) Download the application form below, print it out or Contact Us.

  2) Fill in all your child’s information and all of the parent/carer information required.
  3) Photocopy your ID. Please refer to the ID guidelines for further information about this requirement.
  4) Attach a passport photo of your child to the registration. Please refer to the photo guidelines for further information about photos.
  5) Complete your credit card details.
  6) Tick off the checklist and sign the declaration at the end of your application form.
  7) Post your application form complete with a copy of your identification, checklist/declaration and the photo to:
