Carer Assist Postal Registrations

If registering online isn’t for you, you can always fill out an application form by following these steps.

• 1) Download the application form below and print it out, or Contact Us and we will post one to you.

  2) Fill in the care recipient's information and all of the carer information required.

• 3) Photocopy your ID. Please refer to the ID guidelines for further information about this requirement.
  4) Attach a passport photo of the person you care for to the registration. Please refer to the photo guidelines for further information about photos.
  5) Complete your credit card details.
  6) Tick off the checklist and sign the declaration at the end of your application form.
  7) Post your application form, complete with checklist/declaration, copy of your identification, and the photo to:
