Carer Assist Postal Applications

If registering online isn’t for you, you can always fill out a traditional form by following these steps.

  • 1) Download the application form (see below) and print it out or Contact Us.

    2) Fill in all of the information of the person being cared for and all of the parent/carer information required.
    3) Make a photocopy of your ID. Please refer to the ID guidelines for further information about this requirement.
    4) Add a passport photo of the person you are registering. Please refer to the photo guidelines for further information about photos.
    5) Complete your credit card details.
    6) Tick off the checklist and sign the declaration at the end of your application form.

    7) Post photocopy of identification, checklist/declaration and the photo to:
